BOTOX® Cosmetic Neuromodulator Injections at Capital Skin Medical Spa

Capital Skin Medical Spa offers BOTOX® Cosmetic (OnabotulinumtoxinA) neuromodulator injections in Clifton Park, NY — the FDA-approved botulinum toxin type A wrinkle relaxer administered directly by Dr. John M. Purcell Jr., MD. Derived from a highly purified protein produced by the anaerobic bacterium Clostridium botulinum, OnabotulinumtoxinA is the most extensively studied cosmetic injectable in history — FDA-approved since 2002 with over 574 peer-reviewed clinical studies. Unlike many local medspas where injections are delegated to nurses or nurse practitioners, every neuromodulator treatment at Capital Skin is performed by our physician owner. Dr. Purcell’s 20+ years of experience as a physician (MD) means precise dosing, anatomically informed placement, and natural-looking results — not a frozen face.

BOTOX® Cosmetic — The Gold Standard Neuromodulator

BOTOX® Cosmetic by Allergan, an AbbVie company, temporarily relaxes the muscles that cause dynamic wrinkles. The OnabotulinumtoxinA molecule blocks acetylcholine release at the neuromuscular junction, preventing contraction without affecting sensation. Results begin in 24–48 hours, reach full effect at 30 days, and typically last 3–4 months. FDA-approved treatment areas include frown lines (glabellar lines), crow’s feet, forehead lines, and neck bands. As an authorized Allergan provider, Capital Skin patients earn Allē loyalty rewards points on every treatment, redeemable for savings on future visits.

Xeomin (IncobotulinumtoxinA) is also available at Capital Skin upon patient request — typically recommended for patients with established neurotoxin resistance. Ask Dr. Purcell at your consultation.

Your Treatment Experience

Consultation

Your journey begins with a one-on-one consultation with Dr. Purcell Jr., MD. He evaluates your facial anatomy, discusses your aesthetic goals, and recommends the right neurotoxin (BOTOX® Cosmetic or Xeomin) and treatment plan for your unique anatomy. Honest, physician-led guidance — no pressure, no upsells.

Treatment

Most neuromodulator treatments take 10–15 minutes. Dr. Purcell uses ultra-fine needles and precise anatomical placement to inject the neurotoxin into targeted muscles. Most patients report minimal discomfort — a slight pinch at each injection site. Topical numbing cream is available on request.

Aftercare

No downtime required — return to normal activities right away. For the first 4 hours: avoid lying flat, strenuous exercise, and rubbing the treated area. For 24 hours: skip heat exposure (hot tubs, saunas), alcohol, and skin treatments. Full written aftercare instructions provided at your appointment.

Results

First effects appear within 24–48 hours as the neurotoxin begins to relax targeted muscles. Full results at Day 30. Most patients enjoy results for 3–4 months. With regular treatments, many patients find they need fewer units over time as muscles gradually respond to consistent relaxation.

What Are Neuromodulators? How BOTOX® Cosmetic Smooths Wrinkles

Neuromodulators — also called wrinkle relaxers or neurotoxin injections — are FDA-approved injectable treatments that temporarily relax the facial muscles responsible for expression lines. BOTOX® Cosmetic (OnabotulinumtoxinA) is the gold-standard neuromodulator at Capital Skin Medical Spa, manufactured by Allergan, an AbbVie company. Its active ingredient is a highly purified protein derived from the anaerobic bacterium Clostridium botulinum — specifically the botulinum toxin type A serotype, the same molecule cleared by the FDA for cosmetic use since 2002. At Capital Skin, Dr. Purcell administers BOTOX® Cosmetic as the standard neuromodulator for all new and existing patients.

The Science Behind the Results: How BOTOX® Cosmetic Works

Wrinkle relaxers don’t fill lines — they stop the muscle movement that creates them. Every time you squint, frown, or raise your eyebrows, a chemical called acetylcholine is released at the neuromuscular junction — the microscopic gap between a nerve and a muscle fiber. The OnabotulinumtoxinA molecule (derived from Clostridium botulinum) works by cleaving SNAP-25 — a synaptosomal-associated protein essential for acetylcholine vesicle release. Without acetylcholine, no contraction occurs. The muscle relaxes. The line softens.

Dynamic wrinkles — the 11s between your brows, horizontal forehead lines, and crow’s feet — form from repeated muscle movement and respond beautifully to neuromodulators. Static wrinkles caused by volume loss are better addressed with dermal fillers. Dr. Purcell assesses both at your consultation so you’re never sold something your face doesn’t need.

Most patients notice initial softening at 24–48 hours. Full effect develops over 10–14 days. Duration is typically 3–4 months, though first-time patients sometimes see results extend to 5 months. Book a complimentary consultation or call (518) 458-7546.

BOTOX® Cosmetic: Our Standard Treatment — Xeomin Available on Request

BOTOX® Cosmetic (Allergan/AbbVie) is the most studied cosmetic injectable in history — over 20 years of post-approval use, approximately 7–8 million treatments performed in the US annually, and over 574 peer-reviewed clinical studies. It is Capital Skin’s standard neuromodulator for all new patients and the overwhelming majority of existing patients. BOTOX® Cosmetic patients are also enrolled in the Allē rewards program — Allergan’s loyalty platform for treatment discounts — at no additional cost.

For patients who have been receiving Botox for many years and notice it wearing off more quickly than it once did, Dr. Purcell may recommend Xeomin (IncobotulinumtoxinA, Merz Aesthetics) as a clinical alternative. Xeomin is a “naked toxin” formulation — just the 150 kDa pure neurotoxin, with no accessory proteins. Approximately 1–3% of long-term Botox patients develop neutralizing antibodies over time; because Xeomin contains zero accessory proteins, it presents a lower immunogenicity profile for this specific population. Xeomin is available at Capital Skin upon patient request or Dr. Purcell’s recommendation — it is not offered as a standard first-treatment option.

Dr. Purcell’s perspective: “For new patients, I recommend BOTOX® Cosmetic without hesitation. The safety record is unmatched, the Allē rewards program adds real value for patients, and the results are predictable. Xeomin is a smart clinical tool I reserve for patients with a specific need — those who’ve built antibody-mediated resistance after years of treatment. The fact that we can offer both means no patient ever reaches a dead end.” Schedule your consultation.

Who Is a Good Candidate for BOTOX® Cosmetic Neuromodulator Treatment?

Most healthy adults 18 and older bothered by dynamic wrinkles are good candidates. Candidacy considerations Dr. Purcell reviews at consultation:

  • Good candidates: Adults with forehead lines, 11s, crow’s feet, or other dynamic wrinkles; preventive treatment patients in their late 20s–30s; patients seeking masseter reduction, lip flip, or neck band treatment; hyperhidrosis (excessive sweating) patients — BOTOX® has FDA approval for axillary hyperhidrosis since 2004
  • Not ideal or contraindicated: Pregnant or breastfeeding patients; known allergy to botulinum toxin or human albumin; patients with myasthenia gravis, Lambert-Eaton syndrome, or ALS; active infection at the injection site
  • Medication review: Certain blood thinners, muscle relaxants, and aminoglycoside antibiotics can potentiate botulinum toxin effects — Dr. Purcell reviews your full medication list before any treatment

Frequently Asked Questions

At Capital Skin Medical Spa, BOTOX® Cosmetic is priced at $14 per unit. Typical treatments range from 20–50 units depending on the areas treated. A full 3-area treatment (forehead + frown lines + crow’s feet) typically requires 37–79 units. As an authorized Allergan provider, Capital Skin patients earn Allē loyalty rewards points on every treatment — redeemable for savings on future visits. Dr. Purcell provides a personalized unit estimate at your complimentary consultation so you always know what to expect before treatment begins.

BOTOX® Cosmetic (OnabotulinumtoxinA, by Allergan/AbbVie) is Capital Skin’s standard neuromodulator — the most clinically studied cosmetic injectable in history with FDA approval dating to 2002, over 574 peer-reviewed studies, and approximately 7–8 million US treatments performed annually. For the vast majority of patients, BOTOX® Cosmetic delivers excellent, predictable results. Xeomin (IncobotulinumtoxinA, by Merz) is a “naked toxin” formulation available at Capital Skin upon patient request. Because it contains no accessory proteins, it presents a lower immunogenicity profile — making it a clinical option Dr. Purcell may recommend for long-term patients who notice their Botox wearing off faster than it once did. Both are FDA-approved botulinum toxin type A neuromodulators. Dr. Purcell will help you determine the right choice at your consultation.

Unit requirements vary by treatment area and individual anatomy. Forehead lines: typically 10–20 units. Frown lines (11s): 15–25 units. Crow’s feet: 10–15 units per side. A complete 3-area treatment typically requires 37–79 units total. Men often need 20–40% more units than women due to muscle mass differences. Dr. Purcell assesses your anatomy at consultation and provides a personalized recommendation — never one-size-fits-all dosing.

BOTOX® Cosmetic results typically last 3–4 months. Results begin to appear within 24–48 hours, with full effect visible at Day 30. First-time patients may notice results wearing off slightly sooner; with consistent treatment every 3–4 months, many patients find their results last longer as targeted muscles gradually weaken with regular relaxation.

Safety depends primarily on the injector’s qualifications, not the facility type. At Capital Skin Medical Spa, every BOTOX® Cosmetic treatment is administered directly by Dr. John Purcell Jr., MD — a physician with 20+ years of experience as a physician (MD) and residency training at Albany Medical Center. We do not delegate injections to nurses or nurse practitioners. This physician-supervised, physician-administered approach represents the highest standard of care for neurotoxin treatments.

Dr. John M. Purcell Jr., MD — Owner and Medical Director of Capital Skin Medical Spa — personally administers every BOTOX® Cosmetic treatment. Dr. Purcell is a fourth-generation physician with 20+ years of experience as a physician (MD), including research at the University of Houston Medical Center and residency at Albany Medical Center. He does not delegate injectable treatments to non-physician providers.

For the first 4 hours: avoid lying flat, strenuous exercise, and rubbing or massaging the treated area. For the first 24 hours: avoid heat exposure (hot tubs, saunas, hot yoga), alcohol, and skin treatments (facials, chemical peels). You may apply makeup gently after 4 hours. Most patients return to normal activities immediately. Full written aftercare instructions are provided at your appointment.

Yes — these are widely performed uses of botulinum toxin type A. Masseter injections can reduce jaw clenching, teeth grinding (bruxism), and create a subtle jaw-slimming effect. Botox for hyperhidrosis (excessive underarm sweating) is FDA-approved since 2004 and can provide relief for 6+ months. The Botox lip flip uses 2–6 units along the upper lip border for subtle fullness without dermal filler. Ask Dr. Purcell if these advanced treatments are appropriate for you.
