The Science Behind the Results: How BOTOX® Cosmetic Works

Wrinkle relaxers don’t fill lines — they stop the muscle movement that creates them. Every time you squint, frown, or raise your eyebrows, a chemical called acetylcholine is released at the neuromuscular junction — the microscopic gap between a nerve and a muscle fiber. The OnabotulinumtoxinA molecule (derived from Clostridium botulinum) works by cleaving SNAP-25 — a synaptosomal-associated protein essential for acetylcholine vesicle release. Without acetylcholine, no contraction occurs. The muscle relaxes. The line softens.

Dynamic wrinkles — the 11s between your brows, horizontal forehead lines, and crow’s feet — form from repeated muscle movement and respond beautifully to neuromodulators. For patients who have been receiving Botox for many years and notice it wearing off more quickly than it once did, Dr. Purcell may recommend Xeomin (IncobotulinumtoxinA, Merz Aesthetics) as a clinical alternative. Xeomin is a “naked toxin” formulation — just the 150 kDa pure neurotoxin, with no accessory proteins. Approximately 1–3% of long-term Botox patients develop neutralizing antibodies over time; because Xeomin contains zero accessory proteins, it presents a lower immunogenicity profile for this specific population.
